Two years into his administration, Tittl's biggest challenges have been hiring and retaining good people. In his first year, the agency hired nearly 30 staff members, and he expects to hit the same mark in 2019. He has more than 60 people on his payroll now. He also plans to expand the department's deputy commissioner position from its current size of about seven to about 30 people. While Tittl acknowledges that budget cuts have caused staffing shortages, he also acknowledges that people often choose to work for government agencies because they feel valued. "There are a lot of great reasons why people go to work for the (health department) and there are a lot of good reasons why people stay," he said.

A Health Department is a governmental organization responsible for maintaining a system of public health. They administer the Arkansas Revised Statute 8-58-0216, which provides the framework for how they are funded and organized. The Arkansas Department of Health is one of nine state agencies that directly or indirectly includes the Division of Health Service Regulation, Division of Child and Family Services, Division of Healthcare Quality, Division of Hospital, Division of Drug and Alcohol Programs, Division of Tobacco Control, and Division of Planning, Policy and Program Administration. The Arkansas Department of Health oversees the department. There are 45 counties and the cities of Little Rock and Fayetteville. Cities are independent from the counties they are in.
